Nobody told me when I started this job that sleep deprivation was part of the job description.

Not in those words. But after your first year working 24-on/48-off, running calls through the night, catching an hour here and there, reporting back before your body finished recovering from the last shift, you understand it. This isn't a temporary inconvenience. It's the permanent condition of the work.

What took me longer to understand was what chronic sleep deprivation was actually doing to my body. Not just the fatigue, everyone knows about the fatigue. The deeper stuff: what it was doing to my hormones, my physical capacity, my recovery between shifts, and the long-term cost of running at a deficit for years.

After 21 years, I've learned what helps and what doesn't. More importantly, I've learned that most of what the wellness industry says about sleep is written for people who can choose when they sleep. For first responders, military personnel, and anyone whose schedule is set by someone else's emergency,  the conversation has to start somewhere different.

What Sleep Deprivation Actually Does to Physical Performance

This isn't about feeling tired. Sleep deprivation produces measurable, documented physiological changes that directly impair physical performance. Understanding the mechanism matters because it tells you what to target.

Glycogen replenishment is impaired. During sleep, your body restores muscle glycogen, the fuel that powers anaerobic and high-intensity aerobic efforts. Without adequate sleep, that restoration is incomplete. You show up to the next shift or the next training session with partially depleted fuel stores, regardless of what you ate.

Testosterone drops. Cortisol rises. Sleep is when the majority of daily testosterone production occurs. Reduce the sleep, reduce the output. Meanwhile, sleep deprivation keeps cortisol, the stress hormone, chronically elevated. The relationship between cortisol and testosterone is oppositional: when one goes up, the other comes down. Immune function drops. Sleep is a core component of immune regulation. Chronically sleep-deprived first responders are more susceptible to illness, slower to recover from minor injuries, and more vulnerable to the inflammatory conditions that accumulate over a long career in high-demand occupational roles.

The takeaway: sleep deprivation isn't just fatigue. It's a systemic physiological state that affects fuel availability, hormone balance, tissue repair, coordination, and immune function all at once.

What You Can Control

You can't always control when you sleep. You can control the quality of sleep when it happens, what you put in your body before and after shifts, and how you support the systems sleep deprivation most directly attacks.
